General Information & Guidelines
LOCATION
Unless otherwise indicated, all courses are held at the University of Southern California University Park Campus and USC School of Dentistry, 925 West 34th Street, Los Angeles, California. Room assignment and information will be indicated at the time of registration. The only entrance of the dental school open to the public is the main entrance located on 34th street.

REGISTRATION METHODS
All registrations are processed in the order in which they are received and on a space available basis. We suggest early registration for limited attendance courses and hands-on workshops. In order to secure a place in a course, a completed registration form accompanied by a check made payable to USC School of Dentistry must be received for each participant. Visa and MasterCard are also acceptable but we cannot accept cash. No reservations may be made without full tuition payment. Please complete separate registration forms and make separate payments for each course registration. For your convenience, you can use one of the following options:
Registrations will be acknowledged by mail (if time permits), fax or electronically through e-mail. If you mail your registration, please allow sufficient time for your check to reach us to qualify for early registration. It is necessary to allow three extra days due to the fact that all mail must first go through the University mail system prior to reaching our office.

CHECK-IN
On the day of the course our Registration and Hospitality Desk will open one hour prior to the course start time. When courses are held at USC School of Dentistry, the Registration & Hospitality Desk will be located in the East Lobby of the first floor (click for location). Please arrive at least half an hour before the start of each course to sign in and pick up course materials. You will then be directed to the appropriate clinic or lecture room. At-the-door registration is permissible for courses where space is available. We advise contacting the Office of Continuing Education 48 hours prior to the course to ensure space availability.

PARKING
The USC School of Dentistry (Norris Dental Science Center)
is located at 925 W. 34th Street, next to Gate 5 at the intersection
of Jefferson Blvd and McClintock Ave. The closest parking
garages are listed below and accessible at a fee of $8 for
all-day multiple-entry parking:
- Lot
1 : Close to the dental school. Entrance is through
Gate 8 on Jefferson Blvd., between Vermont Ave. and McClintock
Ave. Open Monday - Friday only. (Pay station at $1 per hour)
- Parking Structure A: Entrance is through Gate 6 on Vermont Ave. and 36th St. Open Monday - Friday only.
- Parking Structure D: Entrance is through Gate 4 on Jefferson Blvd and Royal Street. Open Monday - Friday only.
- Parking Lot V: Located at the back of the dental school. Entrance is through Gate 5 on McClintock Ave. Open for guests on Saturday and Sunday only.
- Parking
Lot M: Located at rear right of the dental school.
Entrance is through Gate 5 on McClintock Ave. Open for guests
on Saturday and Sunday only.

CREDITS
The USC School of Dentistry is an ADA CERP, CDA and AGD recognized provider. Courses receive credit units according to guidelines of the California State Board Dental Examiners on an hour-for-hour basis. Please refer to each course listing for the number of credit units awarded for a particular category. Certified dental assistants should contact the Dental Assisting National Board regarding their continuing education credit guidelines and course requirements. Upon completion of each course, participants will receive a verification of attendance, indicating the credit hours awarded. Continuing education credits awarded may not apply toward license renewal in all states. Course participants are responsible for maintaining their own records for license renewal and forwarding their course credit information to the appropriate licensure board(s).

REFUNDS
Refunds are granted only if a written notification is received at least 14 business days prior to lecture courses and 21 days before hands-on and limited attendance courses. 50% of the tuition minus processing fee will be refunded if cancellation occurs 7 business days prior to a lecture course and 14 business days prior to hands-on or limited-attendance courses. No refund is granted afterwards. A $60 fee is withheld for processing all refunds. If insufficient enrollment necessitates cancellation of a course, all tuition fees will be refunded.
The USC School of Dentistry cannot be responsible for the refund of any part of the tuition as a result of emergencies, unforeseen circumstances, or events beyond its control. Also, the USC School of Dentistry is under no financial obligation to reimburse participants for airline tickets, hotel reservations, or any other costs incurred should the need to cancel or reschedule programs arise.

PARTICIPATION
All qualified individuals shall have equal access to participate in the USC School of Dentistry Continuing Education programs and facilities regardless of race, color, creed, religion, national origin, sex, sexual orientation, age, marital status, disability, public assistance status or veteran status.

STUDENT ENROLLMENT
USC School of Dentistry students are allowed to attend most continuing education courses that are not hands-on or limited-attendance courses. Students are required to complete an application form for each course and submit it to the Office of Continuing Education. Attendance is based on space availability and a nominal fee will be charged.

TAX DEDUCTION
The IRS allows an income tax deduction for continuing education course expenses when course content is designated to maintain or improve skills required for an individual's business. Deductible expenses may include, among others, registration fees, cost of travel, meals and lodging. Please consult with your tax advisor or accountant.

Sponsorship of a continuing education course by the USC School of Dentistry does not imply subject matter, product, procedure or technique endorsement. Courses may include subject matter of an experimental and/or controversial nature and commercial references. This material is offered to the profession for educational and informational purposes in a spirit of academic freedom. In addition, advertisement in this publication does not equate to an endorsement or preference for any product or procedure. Participants are also advised that potential risks to patients may exist when using limited knowledge while integrating new techniques into their practice.
The USC School of Dentistry reserves the right to cancel any course, limit enrollment, modify the announced course content, or change the location, time, date and faculty of any course as may be deemed necessary or advisable. Should a significant change occur, every effort will be made to notify registered participants in advance.
The USC School of Dentistry is not responsible for providing liability or malpractice insurance in workshop courses involving patients. Participants enrolled in courses which involve patients are required to provide proof of active dental license, CPR certification and insurance coverage.
Registration in any continuing education program implies permission for photographs to be taken and used in future publications.
